These plans combine an Alliance Select PPO qualified, high-deductible health plan (QHDHP) with a health savings account. Use your HSA account to pay for qualified medical expenses, such as office visits, prescription drugs, dental expenses, and laboratory tests. The money you put into your account may reduce your income taxes for the year and grows tax-free (state tax treatment may vary) — similar to money saved in a 401(k) plan.

• Contraceptive — Covers oral contraceptives and contraceptive devices that are used to prevent pregnancy

• Life Insurance — Low-cost insurance available through USAble Life Insurance Company. Benefit amounts start at $10,000, and premium rates are determined by age and gender
• Vision — 10% to 60% discount on eyeglasses, contact lenses, non-prescription sunglasses, and other vision items. Discounts on eye exams available in many areas. Offered through Avesis, which contracts with more than 10,000 retail optical outlets nationwide
• Maternity — Covers services related to pregnancy and delivery, including prenatal services normally associated with pregnancy. The $2,500 deductible is in addition to your health deductible, but offers you coverage for the costs of pregnancy.
